Interproximal early carious lesions in a high caries risk patient

Patient Presentation and examination: 

Personal information and relevant history: 30-year-old male, , with a personal history of frequent consumption of sugar-rich and/or acidic foods and beverages and a past dental history of  extensive caries (high risk), 

Routine radiographic examination: Multiple small radiolucencies were noticed on the mesial surface of 31, the mesial and distal surfaces of 30, and the distal surface of 29 (Figure 1)

Diagnosis: Based on the radiographic examination, the diagnosis was as follows:

#29 (distal): Early caries extending to the outer third of dentin (D1)

#30 (mesial and distal): Early caries extending to the outer third of dentin (D1)

#31 (mesial): Early caries extending to the outer third of dentin (D1)

Treatment: 

  • The lesions were treated with one application of Curodont Repair Fluoride Plus
  • The patient was asked to use a prescription-strength fluoride (5000 PPM) toothpaste at home. 
  • Oral hygiene instructions were reinforced.

Follow-up: At the 8-month follow up, the treated lesions showed lack of caries progression and reduction in lesions sizes. (Figure 2)

Take-away: Patients with a history of multiple carious teeth and restorations require regular recalls for early diagnosis of caries so as to enable early intervention.
